  1. All email correspondence sign-ups must be linked to an active email account that is the email account of the person signing up.
  2. Existing customers may participate by signing up for new or additional email notices.
  3. One winner will be randomly chosen from all participants that have signed up for new email notices between the sweepstakes dates of November 2, 2009 and December 14, 2009.
  4. The winner will be notified via email and announced on www.ceiva.com and in the CEIVA newsletter.
  5. By entering, you give CEIVA® the permission to publish your name in the CEIVA® Newsletter, e-mails, website, printed materials and any other promotional materials distributed by CEIVA Logic, Inc.
  6. The Email Sign up Sweepstakes is open only to persons who are eighteen (18) years of age or older.
  7. The Contest is void where prohibited or restricted by law.
  8. Employees of CEIVA® and its related companies, agents, and suppliers, as well as corporate and government accounts with volume-purchase-level agreements are not eligible to participate.
  9. CEIVA is not responsible for late, garbled, lost, misrouted or scrambled transmissions or incomplete entries; failed computer hardware or software, telephone, server or other technical failures. CEIVA® reserves the right to modify, terminate or suspend this Sweepstakes should viruses, bugs, unauthorized human intervention or other causes beyond the reasonable control of CEIVA corrupt or impair the administration, security, fairness or proper play of this Sweepstakes.
  10. The Email Sign Up Sweepstakes winner will be selected based on one random selection from all that entered. The decision will be final and not open to review or negotiation.
  11. The CEIVA staff reserves all right to refuse any Email Sign-up Sweepstakes submission that does not follow the guidelines of the program.
  12. Prize is not transferable or redeemable for cash.
  13. These Sweepstakes rules shall inure to the benefit of and be binding upon the parties and their heirs, representatives, successors, and assigns.
  14. By entering the Sweepstakes, entrant grants the CEIVA staff the right to publish entrant’s name. Entrants represent and warrant that their name is original to them, and does not infringe upon the rights of any third party.
  15. Each entrant acknowledges that he or she has read, understands and will abide by all official rules.
  16. The decision of CEIVA Logic, Inc. in all matters relating to the rules and administration of the Sweepstakes shall be final.
  17. Drawing will be held on December 21, 2009. The odds of winning will be directly proportional to the number of entries received.
  18. To enter drawing without signing up for the CEIVA mailing list, hand-print your name, date of birth, address, city, state, zip code, day and night phone numbers (with area codes), valid e-mail address and the words "CEIVA Twin Pack Sweepstakes" on a plain piece of paper no larger than 8 1/2" x 11", and insert such piece of paper into an envelope and mail it to CEIVA Logic, Inc., 214 E. Magnolia Blvd., Burbank, CA 91502. Mail entries must be postmarked by December 14, 2009 and received by Sponsor by 1:00 PM PT on December 18, 2009. Limit one (1) entry per person. Limit one (1) entry per outer envelope and each entry must be submitted separately. Photocopies, mechanically reproduced or computer generated entry forms or entry pieces of paper are ineligible and will be disqualified. Entries that are late, illegible, incomplete, unreadable, damaged, destroyed, delayed, lost, tampered with, incomprehensible, forged, mutilated, misdirected, inaccurate, unintelligible or otherwise not in compliance with these Official Rules will be disqualified. All entries become the property of CEIVA and will not be acknowledged or returned. All requested information on the entry card or piece of paper must be completed in English to enter and be eligible. Proof of entering information on an entry card or of mailing such card is not considered proof of delivery to or receipt by CEIVA of such entry.
